Fees for Self-pay Clients

We are pleased to provide Peer Health Assistance services to multiple health professionals. Individuals who are not covered by a statutorily created program funded by license fees may access services on a self-pay basis.

Who may access services on a self-pay basis?

  • Nursing license is expired, revoked/relinquished, or otherwise do not have a license.

  • Pharmacy or Pharmacy Intern license is expired, revoked/relinquished, or otherwise do not have a license.

  • Dental license is expired, revoked/relinquished, or otherwise do not have a license.

  • Dental Assistant or Dental Hygienist

  • Veterinary license is expired, revoked/relinquished, or otherwise do not have a license.

  • Veterinary Technician or Veterinary Technologist

  • Mental Health professional license is expired, revoked/relinquished, or otherwise do not have a license.

  • Student of nursing, dentistry, pharmacy, veterinary medicine or mental health professions

  • Other Professionals including anyone licensed, certified, or registered with the Department of Regulatory Agencies.


Fees

Evaluation Fee

A $1,500.00 one-time fee is charged at the time of service that may be paid by cash, check, money order, or credit card.


Monitoring Fee

A $50.00 monthly monitoring fee is charged for comprehensive case management that includes records management, return to work agreements, consultation with therapist and employer, monitoring compliance, and reporting to regulatory boards as described in the signed rehabilitation contract. The monitoring fee may be paid by cash, check, money order, or credit card.

Monthly monitoring fees are collected through the RecoveryTrek portal.
